OVERVIEW: Children with special health care needs and their families
need a broad array of health and education services to support optimal
health and development. Obtaining needed services is facilitated when
a high level of interagency collaboration exists. The CHOICES project
is developing interagency linkages between Shriners Hospitals for Children,
state Title V Children with Special Health Care Needs agencies, Vocational
Rehabilitation, and Early Intervention programs. Through the collaborative
program, access barriers are removed, interagency service coordination
is improved, and resources are used efficiently. Families receive expert
inpatient and community-based services and assistance with transitions
in health care and education.
