One-Hour Phone Call

"I'm running out of time! " I don't know where to begin.  Transition is really scary! Is something that we hear a lot at HRTW trainings for families and youth who have come to learn about transition from pediatric to adult health care systems and services.  Most have had several years to get ready, but have never really been encouraged to take those new steps that promote self differentiation from their child, steps that prepare families for changing roles as a parent of a child with special health care needs to a parent of an adult child with special health care needs. Yes, the dynamic is different. It is true some youth will require different levels of on going support, but have families been prepared to get ready to "raise and launch an adult?"
 
In this one hour interactive discussion, among seasoned family leaders, a young adult with SHCN, and a family leader in the midst of planning transition for her daughter, we share a new way of accelerated learning and training for jump starting preparation at home and developing the knowledge (K) and skills (S) that are practiced and mastered and demonstrated as abilities (A)—or the KSAs for successful transition.

Join in the dialogue as we describe how HRTW is assisting family leaders to “walk the talk” while also identifying transferable skills for training and supporting CYSHCN and their families.

During this topical call:

  1. The learner will state the rationale behind the importance of addressing and acknowledging anxieties and fears of families before teaching transition skills that are transferable to youth.

  2. The learner will be able to identify differences between Knowledge, Skills, and Abilities as they relate to preparing for practice and mastery of essential skills for health care transition to adulthood for CYSHCN.

  3. The learner will identify lessons learned, apply it to their personal journey with their child and family, and incorporate changes in work activities and functions that support other families and CYSHCN.
